When you submit an intake form or referral form, we collect:

• Business information: clinic name, legal structure, province, years in operation, ownership type, revenue range
• Equipment details: device type, condition, purchase price, vendor information
• Credit information: self-described credit profile, bankruptcy history, down payment availability
• Revenue plan: treatments offered, pricing, marketing strategy
• Contact information: name, email, phone, best time to call
• Marketing attribution: UTM parameters, referring URL, source page

2. How we use your information

We use your information to assess financing options, contact you about your inquiry, and, with your consent, share it with our funding partner, LeaseVantage, for underwriting purposes.

3. Information sharing with LeaseVantage

When you provide consent on the intake form, we share your submitted information with LeaseVantage, our exclusive funding partner. LeaseVantage uses this information to assess and process financing applications. LeaseVantage is an independent company with its own privacy practices.

We do not sell, rent, or trade your personal information to any other third parties.

4. Data retention

We retain submitted information for the purpose of processing financing inquiries and maintaining referral fee audit records. If you request deletion, we will remove your personal information from our active systems, subject to legal or contractual retention obligations.

5. CASL compliance

We comply with Canada's Anti-Spam Legislation (CASL). You consent to be contacted by email and phone when you submit a form. You can unsubscribe from communications at any time by replying to any email with "unsubscribe" or contacting us directly.

6. Your rights

You have the right to:

• Access the personal information we hold about you

• Request deletion of your personal information
• Withdraw consent for future communications
